By performing any recommendation from Dr. Nate Feehrer, in person or online, I assume full responsibility, during and after participation for my choice to use or apply, at my own risk, any portion of the information or instruction I receive. Heirs, legal guardians and legal representatives of/and Dr. Nate Feehrer, DPT hereby release, waive, and discharge any claims against Dr. Nate Feehrer, DPT, and any of his associates, affiliates, or family.

Part of the risk involved in undertaking any activity or program is relative to my own state of health or wellness and to the awareness, care, and skill with which I conduct myself in that activity or exercise. My choice to participate in any activity or exercise brings with it my assumption of those risks of results stemming from this choice of fitness, health, awareness, care, and skill that I possess and use.
